Invisible text input field when creating new instance (macOS)

Bug

When creating a new instance, the "Please enter a name for the instance." prompt appears but the text input field is completely invisible. Cannot type or see any input.

Reproduction

1. Run cs
2. Press n to create a new instance
3. The prompt text shows but no input field is visible

Attempted fixes (none worked)

• TERM=xterm-256color cs / TERM=xterm cs
• LANG=en_US.UTF-8 cs
• Resizing terminal window
• Reset Ghostty config (default font)
• brew upgrade claude-squad (already on latest)

Environment

• claude-squad: 1.0.17 (Homebrew)
• macOS: 26.3.1 (Build 25D2128, Apple Silicon)
• tmux: 3.6a
• Terminals tested (all reproduce):
  • Ghostty 1.3.1
  • iTerm2
  • macOS Terminal.app
• Shell: fish (via /opt/homebrew/bin/fish)

Logs

No other errors in /var/folders/.../T/claudesquad.log.
