Add TODO.md collating all TODOs from the repository (#2160)

- [x] Understand current TODO.md structure (317 data rows, 5 columns:
File, Line, TODO, Effort, Feasibility, Scope, Recommendation)
- [x] Write script to assess each row via Claude Sonnet 4.6: does this
TODO need further info or human intervention before it can be actioned?
- [x] Run 13 batches of 25 items → 317 assessments (141 Yes, 176 No)
- [x] Apply updated TODO.md with new "Needs Human?" column (Yes/No)
- [x] Commit and push
